Dear NPCR Program Directors and Contacts,
The 2024 NPCR PEI survey opens TODAY, July 1, and will remain open July 1 - July 31, 2024. All
responses are due no later than July 31, 2024, at 5pm EST.

Please use your NPCR-CSS DocServer username and password to access the survey.
For login assistance, contact support@npcrcss.org or call 301-572-0502.

All NPCR registries must complete and submit the 2024 PEI. Only one survey per registry can be
submitted.
Instructions for completing and submitting the 2024 PEI survey are attached. Please review all
instructions prior to starting the survey.
If an extension may be needed, please follow the attached extension request procedure. The deadline
to request an extension is July 22, 2024.
