Revision to the Collection Instrument
Change #1: We are revising the Privacy Act Statements on this collection.
Justification #1: SSA’s Office of Law and Policy is conducting a systematic review of SSA’s Privacy Act Statements on agency forms. As a result, SSA is updating the Privacy Act Statements on this collection.
Change #2: We are revising the language in #5, which incorrectly referred to question #3 instead of question #4.
Justification #2: We are revising to correct a typographical error.
We will make these revisions upon OMB’s approval. These revisions will not affect the current burden on the respondents reflected in #12 of the Supporting Statement.
