(cognitive, diagnostic, aptitude, achievement), survey procedures, interview procedures, or
observation of public behavior (including visual or auditory recording)

You are authorized to implement this study on receipt of this letter.

This project should be conducted in full accordance with all applicable laws and regulations, and
must adhere to the project plan that was reviewed by Solutions Institutional Review Board.

Any changes to the approved study documents must be approved prior to use.
ANY changes to the study, regardless of the study review level (exempt, expedited or full board) must be approved by Solutions IRB prior to
implementation. The study protocol must be followed in detail. If any aspect of the approved protocol is not followed, Solutions IRB may terminate the
study and revoke IRB approval. The Attestation of Responsibility and Signature of Principal Investigator digitally signed by the PI upon submission
adheres (refer to Appendix A).

You must also report any unanticipated problems involving risk to participants or others to Solutions IRB immediately.

Upon any inquiries or reports about the conduct of this study, or on the receipt of any
participant or other complaints, you may be asked to provide documentation to Solutions
Institutional Review Board related to this study and are expected to immediately comply with any
such requests.
Please refer to your sponsor/regulatory office guidelines on timeline to send IRB decision letters,
if applicable.
