Hi Danny,
Please find attached an OMB package for the service delivery GenIC for your review for privacy determination. For a
quick summary, we have a contract with Brunet Garcia to update the MyMobility Plan and its associated products
regarding medications. They will be conducting 5 virtual focus groups to get feedback from adults aged 55-74 (the
intended users of the materials). The contractor will need to collect names and mailing addresses to send the materials

and incentives to participants, but this contact information will be stored separately from the data collected during the
focus groups.
