Dear Applicant,

To help recruit, train, and retain a diverse public health laboratory workforce and advance health equity, the Centers for Disease Control and Prevention (CDC) and the Association of Public Health Laboratories (APHL) are working to ensure that candidates from a wide variety of backgrounds and communities have the opportunity to apply for the Career Pathways in Public Health Laboratory Science Program. APHL and CDC invite you to complete this anonymous survey to gauge promotion and recruitment efforts for the program: <INSERT LINK>. It should take no more than 5 minutes to complete.

This survey is voluntary. You may choose to skip questions by selecting “Prefer not to answer." Regardless of whether you chose to answer the questions, please return to the Career Pathways application portal and type "CONFIRMED" in the text box below to acknowledge that you accessed the survey.

The information collected is anonymous and cannot be linked back to your [Fellowship or Internship or Genomic Data Internship] application. Data will be analyzed and reported only in aggregate for internal use to inform CDC of the promotion and recruitment efforts for the program.

Congratulations! We are so thrilled that you have been selected for the APHL-CDC Public Health Laboratory [Fellowship or Internship or Genomic Data Internship] program and matched with a host laboratory.

To better understand various aspects of diversity of the individuals who were selected to participate in this most recent cohort, APHL and CDC invite you to complete an anonymous and voluntary survey: <INSERT LINK>. You may have filled out the same survey during the application process to help us understand the effectiveness of our promotion and recruitment efforts. However, we need your help again since the de-identified information previously collected cannot be linked back to your application.
