Workforce Acceleration Initiative (WAI) Technical Assistance Follow-up Survey


Introduction

Through the Technical Assistance Program, WAI offers Public Health Agencies (PHAs) and WAI Placements support to increase their capacity to accomplish their systems improvement priorities and goals.

This survey will help the WAI program team learn more about your experience with the Technical Assistance Program. Your responses will be used to improve and strengthen the WAI Learning’s Community.

Responses to this survey will be reviewed by individuals on the CDC Foundation WAI team only. Your responses will be stored in secure locations where individuals outside the WAI CDC Foundation team are unable to access your responses. Your responses will not be shared directly outside of the CDC Foundation without your written permission.
